Annual Recertification Packet

Wait until our office contacts you. Fill out this packet only after Sherman Housing Authority contacts you to schedule your annual recertification. Please do not send it early or on your own. If you are not sure whether your recertification is due, call 903-893-3139.

Fill out your annual recertification packet here before your recertification appointment. When you send it, it goes to our office as one PDF packet, and you get a confirmation number.

Please read the instructions thoroughly

Return to the office at your appointment with the following documents:

Assets

Income

Medical expenses

Only applies to elderly or disabled households. Includes any out-of-pocket medical expenses paid in the last 12 months; anything turned in that is not within the year will not be used. Bring any medical expenses that you have. We will look at them and let you know if we are able to use them.

Documentation needed:

College students

The tenant must report all changes in income or family composition within ten (10) calendar days of such change.

You will have an appointment to turn in your packet. At that appointment we will go over the packet with you and compare it to what was reported last year. At the appointment you will need to have all your documents in order. You are being notified in advance, and you will have enough time to get your documents in order. If you have any questions, call the office at 903-893-3139; we will be more than happy to help you.
